CARRIER BACKGROUND

Delta Dental of Arizona was founded as a non-profit organization by the Arizona State Dental
Association in 1972 and covers more then 450,000 Arizonans through over 1,200 employer
sponsored group dental plans.

Delta Dental's history goes back to the mid-1950's, when the International Longshoremen Workers Union on the West Coast negotiated dental benefits for their members' children in Washington, Oregon, California and Hawaii. Today, the Delta Dental system provides dental benefits in every state, the District of Columbia, Puerto Rico, Guam, and the U.S. Virgin Islands.

Since 1954, Delta Dental has helped improve oral health in the U.S. by emphasizing preventive care and making dental coverage affordable to a wide variety of employers and groups.

COST MANAGEMENT

Delta Dental of Arizona has developed "The Delta Dental Difference". This aggressive 5–step cost management program, which applies to all Delta Dental products includes:

• Dental case management, which is a peer-review system ensuring treatment meets accepted standards of care.
• An extensive network of participating dentists, which allows family members freedom-of-choice when selecting dental care providers, wherever they may live.
