We are pleased to provide a comprehensive level of benefits to our full-time associates as outlined below.
Medical
Medical benefits are provided to associates on the first day of the month following 90 days of employment. Associates may elect coverage for their family.
A High-Deductable Health Plan (HDHP) with a Health Savings Account is also offered.
Dental
Associates are able to experience first-hand the superb service provided by our company. Our dental plan is provided to associates and their families without a premium contribution. Dental benefits are available on the first day of the month following 90 days of employment.
Vision
Vision benefits are provided to associates on the first day of the month following 90 days of employment. Our vision plan is provided to associates without a premium contribution. Associates may also elect coverage for their family.
Flexible Spending Account
Our flexible spending account allows associates to reap a tax advantage when paying for medical or dependent care expenses. Associates have the option of using FSA Debit cards to reduce initial out-of-pocket expenses. New associates are eligible to participate in the plan on the first day of the month following 90 days of employment.
Life Insurance and Accidental Death and Dismemberment
Group term life insurance is provided to all associates. Delta Dental of Arizona pays the full premium for this coverage and associates are eligible to participate on the first day of the month following 90 days of employment. Associates may also purchase additional life insurance for themselves and their family, and premiums are payroll deducted.
Short and Long Term Disability Insurance
Our disability program provides income protection in the event an associate is unable to work because of illness or injury. Delta Dental of Arizona pays the full premium for this coverage and associates are eligible to participate on the first day of the month following 90 days of employment.
401K Retirement Plan
Associates are eligible to participate in our retirement plan on the first day of the calendar quarter, following three months of service and attainment of age 21. Daily valuation and internet access make it easy to monitor and manage retirement dollars. Administration fees are paid by Delta Dental of Arizona and contributions are made on a bi-weekly basis.
Aflac Insurance
Associates may elect this voluntary benefit for themselves and
family members. You may select from Personal Accident Indemnity
Plan, Personal Cancer Indemnity Plan, Personal Sickness Indemnity
Plan and Specified Health Event Protection Plan.
Employee Assistance Program (EAP)
This benefit is available to all associates without a premium contribution.
Vacation Time
Delta Dental of Arizona encourages employees to balance work and play by providing a generous amount of vacation time. Associates accrue vacation hours on a bi-weekly schedule. Associates can cash in up to 40 hours of vacation time each December.
Paid Time Off (PTO)
Our PTO plan allows associates to manage their own occasional absences from the scheduled workday. PTO may be used for personal or family illness, medical or dental appointments, or for personal business. If associates don’t use all their PTO time, they are allowed to cash in up to 48 hours each December.
Holidays
Associates receive 10 paid holidays and one Floating Holiday annually.
