Dental Supply Donation Program (Smile Bags)

Delta Dental of Arizona Foundation provides donations of dental hygiene supplies to help support its mission to create a path to better health and wellness. This program gives dental supplies to nonprofit organizations delivering oral health education to underserved and uninsured populations.

What's in a Smile Bag?

Inside a Smile Bag

Every Smile Bag contains basic dental care necessities to help keep a smile healthy and bright. Typically, Smile Bags include:

  • One toothbrush (infant, junior, youth or adult)
  • Toothpaste (youth or adult)
  • Floss
  • Education card (bilingual English/Spanish)

Oral health instructions, screenings or other dental health services should be provided with distribution.

Donation Guidelines

Our mission is to create a path to better health and wellness. Our Smile Bag Program is just one way we can work toward achieving this mission. While many organizations qualify for a Smile Bag donation, we do have some guidelines. 

Eligible Organizations

  • Charitable organizations improving the health and wellness of underserved Arizonans
  • Schools
  • 501(c)(3) nonprofits
  • Indian tribes
  • Government agencies

Ineligible Requests

  • Promotional or marketing use by dental offices to increase their patient base
  • For-profit entities (exceptions are made when requests are made on behalf of an event serving the community in which it is participating)
  • Third-party requests (i.e. nonprofit organizations requesting donations on behalf of or for another nonprofit or school)
  • Any distribution of Smile Bags without oral health services or education from a dental health professional

Donation Requirements

  • Eligible organizations must reside in and serve communities within the state of Arizona
  • Oral health instruction, screenings or other dental health services are to be provided with distribution of the supplies
  • Requests must be submitted at least one month before the supplies are needed (Large requests require a minimum of six weeks for processing)
  • Organizations within Maricopa County are expected to pick up their donation
  • Complimentary shipping is available to organizations outside of Maricopa County
  • Organizations are responsible for assembling Smile Bags from supplies provided
  • The plastic Smile Bags should be used only for items from Delta Dental of Arizona Foundation
  • Requests of 1,000 or more Smile Bags may be required to provide additional information and documentation
  • Smile Bag donations are granted based on availability at the time of request, and at the sole determination of Delta Dental of Arizona Foundation
  • Item quantities may be adjusted to accommodate packaging or shipping constraints
  • Donations are limited to one request per organization per calendar year
