Enrollment Information

The enrollment change period runs November 15 - November 29, 2023. Any changes to your dental plan should be made by November 29.

  • Employees who are not currently enrolled on a Swift Transportation dental plan but would like Delta Dental coverage for 2024 must enroll by November 29, 2023.
  • Employees who are already on a Swift Transportation dental plan will automatically be enrolled in a Delta Dental plan. If you need to make changes to your plan, please do so by November 29, 2023.

Once the enrollment change period ends, you'll need a qualifying life event to enroll in our dental plans for Swift Transportation.

Much like your car, it wouldn't take much to spend a thousand dollars or more. A crown may cost $953 and a root canal might run you $997. Even a simple cleaning would come in at more than $83. Dentist visits can be expensive, but dental insurance coverage can save you lots of money — especially on routine visits and preventative oral health care.

Preventive care, including regular checkups and cleanings, is the key to maintaining your oral health. With regular visits to the dentist, problems can be diagnosed early and treated without extensive testing or elaborate and expensive procedures — and that keeps the costs of dental care low.

  • Benefit year maximum: $2,500 per person
  • Orthodontic lifetime maximum: $2,000 per person
  • Deductibles: $50 per person/$150 per family
  • Basic Coverage: 80%
  • Major Coverage: 80%

 

Core Plan - Benefit Summary

  • Benefit period: January 1, 2024 to December 31, 2024
  • Benefit year maximum: $1,500 per person
  • Orthodontic lifetime maximum: $1,500 per person to age 19
  • Deductibles: $100 per person/$300 per family
  • Basic Coverage: 80%
  • Major Coverage: 60%

 

Value Plan - Benefit Summary

  • Benefit period: January 1, 2024 to December 31, 2024
  • Benefit year maximum: $1,000 per person
  • Orthodontic lifetime maximum: N/A
  • Deductibles: $100 per person/$300 per family
  • Basic Coverage: 80%
  • Major Coverage: Not covered

If you are new to the plan and sign up during the enrollment change period, you should receive an ID card in the mail by January 1, 2024.

Misplaced your Delta Dental ID card? No problem! Here are three ways to access your ID card:

  1.  Log in to the Delta Dental Mobile App to view your ID card.
  2.  Download and print your ID card from the member portal.
  3.  Call 888.651.3029 to request a new ID card from customer service.

Note: You don't need your Delta Dental ID card to visit the dentist. Your dental office can verify your coverage by asking a few simple questions.
